We offer Veri-5 kits:
                 *Veri-5 is a personal drug screen collection kit that can be used confidentially in your own home.  The five popular drugs tested are: amphetamines (crystal meth), cocaine, marijuana, opiates and PCP. Urine is collected into the provided container with a confidential PIN number and dropped off at the laboratory.  Results can be obtained by phone using the provided PIN number. 
                 *A physician's request is not required for the Veri-5.
                 *NOTE:Veri-5 results are not intended for medical, workplace or law enforcement purposes

        Patients with no insurance: DLS will give a 50% discount if payment is made at the time of service (cash or check payments are  accepted).  The discount can be offered if laboratory personnel is notified at the time of service.

        Out of state requests will be honored with the following required
        written requisition (must have MD name, address, phone #)

        diagnosis
        any special instructions
